I can relate exactly to what you are saying.  I was lucky to learn early on in life to follow that &quot;inspiration.&quot;  Never had a job interview in my life.  My work always &quot;appeared&quot; in relation to what I was focusing on.  Can be a bit scary for those around you sometimes but not for you once you understand how it all works.
My last change was to move from being the chairman of a successful 40 person international management company to opening my hypnotherapy practice some years ago.
Won&#039;t you miss the exotic travel (my work took me to India, Africa, China and Europe), the seeming glamour/status, the money.  Of course I appreciated having experienced those things (after all I had asked for them some years ago) but inside I knew it was time to move on.
How will you get clients? make enough money? Nobody can make a living helping others succeed etc. etc.  Were more of the comments and advice I got.
Well short story is that within months my practice was booming.  Now it takes  months before I can give someone an appointment and everyday is fascinating. 
Moral of the story (it does not sound like you need encouragement) is to follow the inspiration and ignore the fear (self hypnosis can help you do that!).
